We accept all insurance as long as out-of-network benefits are available.  We will do a benefit match to the in-network benefits so the patient is not penalized for going out of their network.  The only exception would be if the patient has an out-of-network deductible amount different from the in-network and it is over $500.00 and not met.  We ask that you call and get our approval before scheduling if this exception is the case.  Please feel free to contact us or 877-567-7874 ext. 7321.

You will be contacted prior to your surgery with an estimated procedure cost for you surgery.  Perimeter does offer non-insured discounts if paid on or prior to the day of surgery.  Please note that these are estimated costs and additional procedures if performed by your surgeon will be billed to you.

SELF PAY – COSMETIC SURGERY  - ELECTIVE SURGERY
Payment in full must be received prior to or on the day of surgery.  We accept cash and all major credit cards. Any additional fees will be billed to you.

If your insurance company is not listed, it may be considered to be part of one of the networks listed above.  Please call our office at 770-551-9944 and ask to talk with the billing department for more information.
